Axial CT (A) shows hypodense areas in the bifrontal periventricular white matter (arrowheads). MRI shows multiple tiny cysts in the periventricular and subcortical white matter of both cerebral hemispheres in association with surrounding T2 hyperintensity. Postcontrast T1W1 and ADC map axial images reveal cystic lesions/enlarged perivascular spaces without enhancement (B, arrowheads) or restricted diffusion (C, arrowheads). Coronal T1-IR (D), coronal T2WI (E), and coronal FLAIR (F) re-demonstrate these lesions (arrows). Coronal FLAIR (F) shows surrounding hyperintensity/gliosis (arrows).
