Extra-adrenal primary CNS paragangliomas are rare neuroendocrine tumors most commonly involving the filum terminale or cauda equina. Hypervascularity is the hallmark imaging feature of paragangliomas, often leading to intense enhancement of the tumor (B, white arrows) and peri-medullary vascular void or enhancement (A , B and C, red arrows), findings that are not seen in myxopapillary ependymoma. Like the ependymoma, paraganglioma can have a “cap sign” due to hemosiderin rim deposition (C, orange arrows). Vertebral scalloping seen in this case is indicating slow growth (A, blue arrows).
