Deep cerebral venous thrombosis is characterized by nonspecific clinical presentations and ischemic infarcts of non-arterial distribution. In this case, the initial non-contrast CT scan showed high density in the internal cerebral veins and the right basal vein (arrow in A), along with low density changes of the right thalamus and lentiform nucleus, as well as small hemorrhages in the right caudate nucleus. MR venography showed non-visualization of the deep cerebral veins (arrows in B), which confirmed the diagnosis of deep venous thrombosis. The clinical condition went downhill rapidly even with timely treatment. A follow-up CT (C) 12 hours after the initial CT scan revealed extensive hemorrhage.
