Intraocular tamponade with silicone oil has been used in the treatment of complicated retinal detachment for years. Intracranial migration of silicone oil to ventricles is a very rare complication. The increased intraocular pressure from silicone oil leads to cavernous degeneration of the proximal optic nerve and subsequent infiltration of the silicone oil into the optic nerve. Deep cupping of the optic disc may also allow the silicone oil to enter the subarachnoid spaces. The CT of this classic case shows hyperdense silicone oil within bilateral eye globes (A, arrows) and similarly attenuated nodular hyperdensities in the bilateral anterior horns of the lateral ventricle (B and C, arrows).
