Prognosis in cases of intracranial aneurysm after incomplete direct operations

Summary

Eleven cases of aneurysm persisting after direct intracranial operation have been reported from a series of 315 aneurysms (274 cases) treated in our clinic. Postoperative angiography is necessary in every case because it reveals whether a clip or ligature is effective or not. An aneurysm remaining after direct operation disappears spontaneously if its neck has been narrowed by a clip or ligature. Reoperation must be considered if the clip has been placed on the aneurysmal body, if the clip has slipped off the aneurysm, or if the residual aneurysm is seen at follow-up angiography to be enlarging.
