These authors evaluated CTA studies of 228 patients paying special attention to the clot features, and correlated these features with early reperfusion rates. Clot features that predicted successful early reperfusion included: shorter clot, residual flow within the clot, and distal location. Reperfusion was achieved in only 8% of patients with longer and proximal clots and in those without residual flow.

Susceptibility-Diffusion Mismatch Predicts Thrombolytic Outcomes: A Retrospective Cohort Study

    American Journal of Neuroradiology November 2014, 35 (11) 2061-2067; DOI: https://doi.org/10.3174/ajnr.A4017

The benefit of thrombolytic therapy was assessed in 54 patients with a mismatch between infarct core as seen on DWI and penumbra as seen on SWI. Favorable outcomes were significantly higher in patients with DWI-SWI mismatch than in those without it. The accuracy of the DWI-SWI mismatch for predicting good outcome was higher than that for the more traditional DWI-perfusion mismatch.
