Zusammenfassung
Die Gefäßmalformationen des Spinalkanals und Rückenmarks sind seltene Erkrankungen, deren mögliche Symptome von transienten neurologischen Ausfällen bis zur akuten Querschnittlähmung reichen. Eine Schädigung des Rückenmarks kann durch Blutungen, Raumforderungswirkung, venöse Kongestion oder “Steal-Effekte” entstehen. Häufiger als die echten (angeborenen) Malformationen, wozu arteriovenöse Malformationen (AVMs), Kavernome und kapilläre Teleangiektasien gerechnet werden, sind die vermutlich erworbenen spinalen duralen arteriovenösen Fisteln (SDAVF) des älteren Patienten, die gesondert dargestellt werden. Die kapillären Teleangiektasien sind meist Zufallsbefunde, können aber differenzialdiagnostische Verwirrung verursachen. Kavernome sind eine wichtige Ursache für Blutungen, wobei sie sich innerhalb eines Hämatoms zunächst der Diagnostik entziehen können. Der MRT kommt im Vorfeld die größte diagnostische Bedeutung zu. Bei Vorliegen einer AVM muss durch selektive spinale Angiographie der Typ des AV-Angioms genau geklärt und über die Wahl der Behandlung entschieden werden. Während Kavernome als “Low-flow-Malformationen” nur operativ zu behandeln sind, kommen bei AVMs prinzipiell endovaskulär-interventionelle, neurochirurgische oder auch beides kombinierende Behandlungen in Betracht. In Einzelfällen kann ein abwartendes Verhalten gerechtfertigt sein.
Abstract
The vascular malformations of the spine and spinal cord are rare diseases. Possible symptoms may consist in a transient neurological deficit, a progressive sensorimotor transverse lesion or an acute para- or tetraplegia. Damage to the spinal cord occurs by bleeding, space-occupying effects and venous congestion, rarely by steal effects. Classification of the true inborn malformations differentiates between arteriovenous malformations (AVMs), cavernomas and capillary teleangiectasias. The more frequent spinal dural arteriovenous fistula (SDAVF) of the elderly patient is a probably aquired lesion which is presented in a seperate paper. Capillary teleangiectasias are mostly incidental findings but may cause differential diagnostic problems. Cavernomas are important causes of hemorrhage and may initially be obscured within the bleeding. MRI is the most relevant imaging procedure in the early diagnostic workup. In case of an AVM selective spinal angiography is required to define the type of the lesion and to decide about the appropriate therapy which may be endovascular-interventional, neurosurgical, combined or attentive.
