Bilateral Dacryops
- Dacryops is a rare clinical phenomenon in which obstruction of lacrimal gland ducts leads to formation of cysts in the lacrimal glands.
- Patients present with painless swelling in the lateral portion of the upper eyelid. Cyst formation is related to periductal inflammation or trauma with secondary duct obstruction due to lacrimal gland hypersecretion and damage to duct walls. Superinfection or hemorrhage may occur.
- Eversion of the upper eyelid reveals a translucent, bluish, fluid-filled lesion in the superotemporal fornix.
- MRI findings: Typically well-defined, small cystic lesions are seen appearing hypointense on T1-weighted imaging and hyperintense of T2-weighted imaging. There is no evidence for any bone erosion.
- Cyst delineation is best accomplished with post contrast T1-weighted images using fat suppression.
- Differential considerations for bilateral lacrimal gland disease include Sjögren's syndrome, Mikulicz's disease, sarcoidosis, and lymphoproliferative diseases.
