Pyogenic Brain Abscess
- Generally solitary, gray-white matter junctions, low T2 capsule, capsule is thin and enhances, high DWI signal, low ADC, relatively low rCBV, MRS shows cytosolic amino acids, acetate, succinate and lipids/lactate
- After successful surgical drainage, they should contain high ADC (recurrence may be seen early as low ADC).
- Presence of neuroinflammatory molecules confer high diffusion anisotropy inside abscess cavity and thus high fractional anisotropy reflects upregulated inflammatory response.
